Welcome. I am a clinical psychologist with extensive training and experience in therapeutic assessment. I provide neuropsychological, psychoeducational, and psychological assessment for children (age 6+), adolescents, and adults. I believe the assessment process should be a validating and empowering experience for each client. From intake to feedback, I will guide you through the assessment process.
Through the assessment experience, I focus on providing clients with useful recommendations and a deeper understanding of the self. I combine standardized assessment instruments to tailor an evaluation unique to the needs of each individual.
An individual or parent may pursue assessment to determine if they or their child is on the autism spectrum, meets criteria for a learning disorder, or is experiencing symptoms of Attention-Deficit/Hyperactivity Disorder (ADHD). An individual may seek out testing to determine if they are experiencing memory impairments or to better understand their emotional or personality functioning.
A Therapeutic Approach to Assessment
I accept Quartz health insurance and private pay
Quartz Health Insurance
If you will be utilizing quartz health insurance, I will check your benefits and inform you of your copay, coinsurance, and deductible responsibilities. I require a credit card be held in my secure system and, prior to the intake appointment, you will be sent a form authorizing me to bill your credit card at the time of service. Please note, copay is billed at the time of service. If your plan requires a coinsurance or deductible payment, your credit card will be billed after insurance processes the claims for rendered services.
Private Pay
If you are paying out-of-pocket, you will be billed $275 for the initial intake appointment and $230 for the feedback session. The time I spend with you completing testing, interpretating results, and writing your report will be billed at $200 an hour. During the intake appointment, I will provide you with the total cost of the evaluation. All payments are due at the time of service. I require a credit card be held in my secure system and, prior to the intake appointment, you will be sent a form authorizing me to bill your credit card at the time of service.
Step 1
If you believe I may be a good fit to provide assessment services to you or your child, please contact me at 608.616.5252 or at drholt@fullcirclepsychologyllc.com to be added to my waitlist. Please leave your name, child’s name if you are calling for your child, and your contact information.
Step 2
When an intake appointment becomes available, I will reach out to you to schedule an initial intake appointment (in-person or virtual depending on your preference).
Step 3
After the initial intake is scheduled, I will send you a link to the client portal. The client portal will contain required forms that will need to be completed prior to the intake appointment. If you are insured under Quartz, I will contact Quartz and check your benefits.
Step 4
Attend an intake appointment (in-person or virtual depending on your preference). This is an hour appointment during which I will gather relevant background information to inform the evaluation and better understand your reasons for seeking evaluation. Based on the information you share with me during the intake appointment, I will develop a tailored evaluation to address your needs. In addition to gathering information to inform the evaluation, the intake is a time to evaluate if we will be a good fit to work together. If either of us is uncomfortable proceeding with the evaluation process, we may choose to not move forward with testing. At the end of the intake, if we both agree to continue with the evaluation process, we will schedule the testing appointment(s) and feedback session.
For child and adolescent intakes: -6-12 parent only
-12+ parent and child attend
Step 5
Attend in-person testing appointment(s). The time it will take to complete testing depends on the type of evaluation and usually ranges from a half day to two days. On the morning of your testing appointment, I ask that you eat breakfast and limit caffeine intake to one cup of coffee. I also ask that you refrain from alcohol and substance use for 24 hours prior to testing and attempt to get a full night sleep the night before your appointment. These steps will ensure I am able to gain an accurate understanding of your brain functioning.
Step 6
Attend a feedback session (in-person or virtual depending on your preference). During this appointment, we will review testing results and diagnostic information. I will provide recommendations and referrals if needed. You will receive a copy of your or your child’s evaluation report. If you would like the report to be sent to a member of your care team (e.g., therapist, psychiatrist, primary care provider, etc.) we will sign releases so that I may send the report for you.
